Toward closed-loop intracranial neurostimulation in obsessive-compulsive disorder

Clinical outcomes of deep brain stimulation for obsessive-compulsive disorder remain limited by open-loop, trial-and-error programming. Here we outline, and provide early evidence toward, closed-loop intracranial neurostimulation for OCD. Using sensing-enabled DBS devices implanted in the ventral capsule/ventral striatum, we characterise candidate electrophysiological biomarkers that covary with symptom state, charting a path to adaptive, symptom-responsive stimulation that adjusts in real time to the patient’s neural and clinical condition.
